The second flight of the Gray Ghost experienced what NASA calls an "anomaly"...
 
At liftoff a fireball shot through the rocket, incinerating the tube that
runs from the engine holder to the parachute compartment
 
Fireball XL-5!
 
The parachute is out below the rocket
and in the path of the engine exhaust!
It was all over in three seconds.


From  the outside, the only damage seen was a cracked fin. After the rocket cooled down the shock cord hardened and crumbled.
